Who are we?

Dr. Eniola Udoyen graduated from the University Of Ibadan in 2000 with a B.Sc in Physiotherapy. She worked with the Nigerian Army at their Army Reference Hospital in Lagos from 2000-2002. In 2002 she enrolled in the School Of Public Health at the University of Illinois at Chicago and graduated in 2004 with a MPH in Health Policy and Management. She resumed her practice of Physical Therapy in 2004 at Schwab Rehabilitation/Mt. Sinai Hospital in Chicago Illinois and has provided Physical Therapy in multiple different settings till date. In 2011 she was awarded a Doctorate of Physical Therapy from Stony Brook University, New York, after she completed the course requirements. She is a Functional Manual Therapist with the Institute of Physical Arts and is working towards her certification. Over that past 21 years Dr. Udoyen has brought healing and relief to innumerable patients in diverse therapeutic settings. The experience she gained over these years helps her continued work to help patient achieve their therapy goals. Some of her patients fondly refer to her as "The therapist with healing hands".

Can I receive therapy in my own home and will Medicare pay for it?

Yes, you can have Physical Therapy, Occupational Therapy and Speech Therapy in your own home and Medicare Part A & B will cover the costs.

Do I have to be homebound in order to receive therapy under Medicare?

Yes under Medicare Part A, However under Medicare Part B, Patients do not have to be home bound.

I am aware that Medicare only pays for limited services, will I be limited under Medicare Part B?

No, Under Medicare Part B, you have unlimited visits as long as the treatments are medically and functionally neccesary and there is documentation to support that.

I have had therapy already this year and I am not sure If I am eligible for more visits under Medicare part B?

I only have private insurance and I will like to be seen by one of your therapists, Is this possible?

Please contact us with your insurance information, we will help you find out if you have out of network coverage benefits or if we are in network with your insurance plan.

I don't have any insurance, can I pay cash?

Yes, we take cash patients.

How long is our therapy session for?

On average, it's a 40-45 minute session however varies on case by case basis depending on the patients medical needs.

I don't want a male therapist, can I request a female therapist?

Yes, you may make your request known when you are contacted by our therapist.

I am a medical provider, how do I refer patients?

Complete and fax a referral form/ script, along with a demographic page which includes patient insurance information. If you do not have referral forms, please call and we will supply them immediately. OR Fax a referral and demographic page to 1 800 920 0624 (i.e., PT / OT/ ST evaluation and treatment, with diagnosis). Our therapist will contact the patient to schedule an initial evaluation after insurance verification is completed.
